Amy T. Cole FNP-BC, CNM-BC, APRN

Welcome! I'm Amy Cole, a dual-certified Family Nurse Practitioner and Nurse-Midwife, and I'm so glad you're here. As a graduate of Vanderbilt University, I founded Cole Family Practice with a clear mission: to create a holistic healthcare home that nurtures your entire family.

With 20+ years of experience in medicine, I've built my practice on a foundation of compassion and excellence. I believe that addressing both physical and emotional health is key to well-being, and I am committed to ensuring that you and your loved ones feel valued and understood at every visit. My goal is to become your trusted partner on your health journey.

My dedication to healthcare has also led me on numerous medical mission trips to serve underserved communities in Central and South America, as well as Haiti. I was humbled to receive the President's Volunteer Service Award for my medical service aboard the US Naval Hospital Comfort Ship, and these experiences continue to enrich my practice today.

Here at Cole Family Practice, I am particularly passionate about offering women the opportunity for holistic midwifery care within our community. My practice serves as a healthcare home for the entire family throughout the lifespan, aiming to address the physical, emotional, and social aspects of health.

 I also find great joy in educating the next generation of healthcare providers by mentoring nurse practitioner interns. I strive to instill in them the importance of holistic care and the critical skill of truly listening to patients—an art that is all too often lost.

When I'm not in the office, I live in Wilson County with my husband, children, and grandbabies. I love gardening, cultivating fresh cut herbs and flowers, traveling, and caring for my chickens.

Amy E. Taylor MSN, APRN, FNP-C

Amy is a native of Nashville with over 20 years of experience in healthcare. She earned her Bachelor of Science in Nursing from Belmont University, and worked as a registered nurse for 9 years prior to returning to graduate school. Her nursing experience includes intensive care, outpatient surgery, and ENT. 

Amy earned her Master of Science in Nursing from Austin Peay State University and is certified through the American Academy of Nurse Practitioners. She has worked in pediatrics the past 10 years providing  compassionate, family-centered care to children from infancy through adolescence. She focuses on building trusting relationships with families while supporting children’s physical, emotional, and developmental health. Known for a gentle approach and clear communication, Amy strives to empower parents with knowledge and confidence in caring for their children. While pediatrics has been her passion, Amy recently sought to expand her practice to encompass adult health as well, working at Belmont University Health Services providing care to students, faculty and staff. 

Amy is married to her high school sweetheart, and together they have two children. Her greatest joys include watching her daughter play volleyball (Union University) and her son play baseball. She also enjoys running, hiking, traveling, and spending time with family and friends. Amy is  honored to care for you and your families!

Sarah Upchurch MSN, APRN, FNP-C

Sarah Upchurch is a board-certified Family Nurse Practitioner. She earned her Bachelor of Science in Nursing from Western Kentucky University. As a registered nurse, she gained experience in cardiac care, intensive care, and case management, experiences that helped shape her passion for providing comprehensive, compassionate care.

She went on to earn her Master of Science in Nursing from Chamberlain University. Since becoming a nurse practitioner, Sarah has primarily practiced adult family medicine throughout eastern and south-central Kentucky.

Sarah believes every patient deserves to feel heard, respected, and comfortable discussing their health concerns. She enjoys partnering with her patients to develop individualized treatment plans that fit their unique needs, goals, and lifestyles. She believes healthcare is never “one size fits all” and strives to create a safe, supportive environment where patients feel empowered to be active participants in their care.

Outside of the office, Sarah lives in Nashville with her fiancé and their 9-year-old cattle dog, Nya. In her free time, she enjoys playing guitar, baking, and traveling.

Russell Cole, CFO

Russ is the chief financial officer for Cole Family Practice. He has over 25 years of experience in the health care industry. He has also been a CPA for 20 + years. He uses his experience to keep the office running smoothly and holds a high standard for the financial integrity of Cole Family Practice. He was born and raised in the East Nashville area and resides in Wilson County with his wife Amy Cole.

Andre Swauncy, MD

Andre Swauncy, MD

Dr. Swauncy is a native of Memphis, Tn. He is board certified by the American Board of Family Medicine. Dr. Swauncy works with Cole Family Practice as our consulting physician. He does not see patients in this practice, but helps ensure excellent patient care.
